To donate a copy of your zine to the library, you will be asked to fill out this donation form. Kaitlyn has physical copies in the library, but you may also decide to use the digital form here -- please do print it out to sign and submit alongside the copy of your zine.
To briefly explain each section of the form:
Section 1: Please provide your best long-term contact information in the rare situation where we will need to contact you in the future. You also need to briefly describe the item -- for example, "Handmade zine created for RELG 216 titled Faith and Disability. You are the creator of the zine, so you will check the first box in the list.
Section 2: This is giving Special Collections permission to make decisions on your behalf regarding the access of your zine. If you are okay with the librarians using our best discretion on your behalf, check the first box. If there is something you specifically do not want researchers to do or if you would like to place any access considerations (for example, allowing/refusing photo copies or allowing/refusing digitization) please check the second box and explain in detail what you would like us to ensure on your behalf, as well as an end-date for when those considerations must be met.
Section 3: This is giving Special Collections permission to make decisions on your behalf regarding the use of your zine. If you are okay with the librarians using our best discretion on your behalf, check the first box. If you would like us to contact you whenever researchers would like to publish work using your donation, check the second box and provide the best way to contact you in the future.
Section 4: In the rare circumstance where Special Collections is removing your donation from their collections (which is especially unlikely for student work), please check the box noting how you would like the removal process to conclude: discarding the donation, returning the donation to you, or something else alongside a description of what you would like us to do.
Finally, you will be asked to sign the form (printed, if possible) and date it.
